David Curtis

Chief Executive Officer, Home Health

Read Biography

David has been an executive leader within Graham Healthcare Group for 15 years. After extensive personal experience with home care delivery to a critically ill loved one in his own home, David’s passion for delivering excellent care and service only deepened: “I was frustrated by the lack of service and responsiveness at times from the nurses, and then there were the nurses whose attention to detail and swift action are the reason my son is alive today.”

  • President of Residential Home Health for more than 13 years, growing the business from to the largest home health care provider in the state of Michigan
  • Managing Partner of Leland Partners, a consulting firm whose clients included General Motors, Philips Electric, Syntel, Inc., the Kauffman Foundation, and Mount Clemens General Hospital
  • Senior Principal with Diamond Technology Partners, a global consulting firm based in Chicago that advised Fortune 500 companies on business strategy & operations

Justin DeWitte

Chief Executive Officer, Hospice

Read Biography

Justin has been an executive leader within Graham Healthcare Group for 10 years. He first worked in palliative medicine at Continuum Geriatric Services (CGS; see below), focused on pain control and optimizing collaboration between Hospice, Palliative Care and Physician Services. Through this work he saw the incredible impact that timely, well-coordinated end-of-life care provides to patients and their families.

Prior to Graham Healthcare Group

  • President of Residential Hospice, a de novo agency he launched and grew to the 2nd largest hospice provider in the state of Michigan.
  • CEO of CGS, one of the largest post-acute physician practices in the Midwest. CGS revolutionized post-acute care through provider excellence and clinical systems tailored to the specific populations served. Upon being acquired by IPC (now Team Health, www.teamhealth.com), CGS became the foundation for their post-acute operations across the country
  • Founder and President, DeWitte Consulting, management consulting firm serving C-Suite executives and principals in healthcare and private equity

Jason Pierce

Chief Financial Officer & Senior Vice President of Accounting & Finance

Read Biography

Jason joined Graham Healthcare Group in April of 2016 as Assistant Controller and was named Controller in March 2018, VP of Accounting and Finance in August 2018, and Chief Financial Officer and Senior Vice President of Accounting and Finance in 2019.

Jason’s roles and responsibilities include oversight of monthly and annual close-of-company financial records; developing GAAP accounting policies and procedures; daily operations of the Finance and Accounting department; managing the preparation of all financial reports, annual budgets, and business plans; and oversight of HR and Payroll.

Tara O’Daniel

Read Biography

Tara joined Graham Healthcare Group in July of 2018. Her roles and responsibilities include management of the Privacy and SOX programs, implementation of process improvement opportunities, project management, staff education around HIPAA, and various internal audits.

LeeAnn Lang

Senior Vice President of Administration

Read Biography

LeeAnn joined Graham Healthcare Group in May of 2016. Looking for opportunity to give back, first as Director of Project Management & Process Improvement, LeeAnn moved into the VP of Administration role in November of 2018.

A Certified Information Systems Auditor (CISA), Project Management Professional (PMP), and Lean Six Sigma professional, LeeAnn’s roles and responsibilities include oversight of back-office functions including Communications and IT, Compliance and Audits, Project Management, Human Resources, and the Recruiting and Marketing departments.

Wendy Bongero

Senior Vice President of Operations, Hospice

Wendy joined Graham Healthcare Group in February of 2012. An experienced Vice President of Clinical Operations with a demonstrated history of working in the hospital and health care industry, Wendy oversees Hospice operations for Residential in Michigan and Illinois. In addition, she leads all Graham Healthcare Group Hospice business units toward standardized processes, policies, and procedure documentation.

Teresa McDaniel

Senior Vice President of Clinical Integration & Performance Improvement

Teresa joined Graham Healthcare Group in 2009 with the Edward Health System Joint Venture. Under her leadership, the home health census grew from 80 to 600 patients and earned top quality (4 star) ratings.

Her roles and responsibilities include development, execution, and ongoing support of home health, hospice, and palliative care clinical integration with health system partners across the system’s full continuum to improve patient experience and outcomes; leading quality and performance improvement efforts for home health, hospice, and palliative care; and developing innovative solutions to support health system initiatives including value-based care and population health.

Certified in Integrated Chronic Care Management, Teresa loves her role because it gives her the opportunity to work across the care continuum with all stakeholders to extend highly reliable care pathways into patient homes.

Teresa brings experience from a wide range of nursing roles in academic and community settings, beginning as a front-line nurse and advancing to nurse manager and then into senior leadership positions.

Natalie Hagyari

Senior Vice President, Home Health Clinical Operations

Natalie joined Graham Healthcare Group in April of 2021. An experienced Vice President of Home Health Operations with a demonstrated history of working within the post-acute healthcare continuum at leading national home health organizations, Natalie oversees Home Health operations for Graham. She also leads all Graham Healthcare Group Home Health business units toward standardization of Home Health clinical operations.
